Use this form to submit coach/trainer certification cost for reimbursement.

Please refer to the Coaching Costs Reimbursement description located in Coach's Corner. Rostered Coaches and Trainers are eligible for 100% reimbursement of fees associated with achieving required certification. Fess above and beyond the required certification are not eligible for reimbursement. Failure to provide the required information and supporting documentation will result in the request being denied and not processed. Submission of the online request form will automatically go to the SMMHA Reps for review. It may take up to 60 days for request to be processed and payment issued. Thank you.
