Player Skills Development Registration Form
As part of the Player Development Program, SMMHA is offering a Shooting Clinic run by TPH. The clinic will be delivered as two (2) sessions and your $50 registration secures your spot in both sessions. The clinic is open to all SMMHA registered players U10 to U18. The sessions are scheduled for October 1st at 4pm and October 22nd at 5pm.
This form has a limit of 30 submissions. At the moment this page loaded, 30 submissions have been submitted. It is possible that not all submissions have been validated at this time, and more openings could become available. However, at this time no more submissions are permitted.
Please Note:

Your completed Player Skills Development registration form AND payment in full via e-transfer is required to secure your spot. Please submit your e-transfer in the full amount of $50 to [email protected]

Once the skills session is full (approx. 20 players), the form will be closed. All registered players will receive a confirmation email.
