Exhibition Game Scheduling, Permit Request Form
Use this form to request an exhibition game (home or away). The form will automatically go to the SMMHA Reps for game permit (only required by OMHA), scheduling, and referee assignment (if a home exhibition game). Reminder, home team pays for the ice and referee costs. Each SMMHA OMHA and OWHA team is eligible to schedule up to 5 home exhibition games without additional cost. If additional exhibition games are requested above 5, the team will be required to prepay for the referee costs. Teams can schedule unlimited number of away exhibition games as long as these do not limit team practice time or regular season game schedule.

EXHIBITION GAME NOTIFICATION

I, Head Coach or Manager, have confirmed and have agreement with the visiting team's Head Coach/Manager the date, time and location of the exhibition game prior to submitting the request form.

I acknowledge that all SMMHA teams are permitted up to 5 home exhibition games prior to incurring additional costs and that any home exhibition game above the 5 permitted, our team will pay the referee costs.
