Use this form to request an exhibition game (home or away). The form will automatically go to the SMMHA Reps for game permit, scheduling, and referee assignment (if a home exhibition game). The home team pays for the ice and referee costs. Each SMMHA OMHA and OWHA team is eligible to schedule up to 5 home exhibition games without additional cost. If additional exhibition games are requested above 5, the team will be required to prepay for the referee costs and an email will be sent to the Head Coach confirming the costs.
