Print Tournament Funds and Permit Request
Complete this form to request SMMHA to pay for a tournament on your team's behalf. SMMHA will process a cheque for each tournament request separately. This form will also be used to notify OMHA/OWHA and initiate a request for travel permit. The Treasurer will notify you when the cheque is ready to be picked up. When you pickup the cheque, a postdated cheque from the team manager/coach dated November 1st (or current date if after November 1st) must be given to the treasurer, or an e-transfer must be sent to [email protected] including Team Name and Tournament Name in the notes section. This will give you time to collect the necessary fees from your players/parents.

  7. Enter the full cost of the tournament unless you have already paid for the tournament yourself then ENTER $0 in this amount. We still need to have the form completed even if you have written the cheque yourself.

    THIS FORM NEEDS TO BE COMPLETED FOR EACH TOURNAMENT YOU ARE ENTERING. 
    IF YOU HAVE ALREADY PAID FOR THE TOURNAMENT YOU STILL NEED TO COMPLETE THIS FORM.  JUST ENTER $0.00 IN THE TOURNAMENT AMOUNT.

    Please write a cheque payable to SMMHA or send an e-transfer to [email protected] for the FULL amount of the tournament registration.  If sending an e-transfer remember to put the team name and the tournament name in the notes section.

    *For any reason there are outstanding balances owing after November 1st your teams practices will be revoked until payment is received. *
