Reminder to ALL Vaccine Eligible Persons to submit proof by Sunday, October 31

In accordance with the OWHA and OMHA policies, the following people need to provide proof of full vaccination (a person is considered fully vaccinated 14 days after their second dose) by October 31:  

- Players born in 2009 or earlier
- Coaches and Bench Staff (including managers, trainers, parent reps, on-ice helpers)
- SMMHA volunteers (including Directors and committee members)
- Game officials
- Instructors


To submit your one-time proof of vaccination to SMMHA association, please click here.  If you are unable to do so online and/or would prefer to do so in person, please come to the PRC on Thursday, October 21st @ 6:30-8:30 pm.

As of November 1, 2021, you will be ineligible to participate in any SMMHA activities until you provide proof or have been granted an accommodation.
