Your Feedback is Important and You Could Win $100


Completing a coach evaluation is important as it helps minor hockey give feedback to current volunteer coaches and helps select and develop the best volunteer coaches to deliver the best quality hockey experience possible.

When completing a coach evaluation, please take the time to provide constructive feedback that reflects your experience and observations in order to help both the coach and the Coaches Committee better understand if the coach delivered on their commitments and is a good fit for the team and minor hockey.

This form is CONFIDENTIAL. The name of the individual completing the form is an optional field and is never disclosed to the coach or the Coaches Committee. The submitted form goes directly to only the 1st Vice President. Names and any identifiers are removed and feedback is incorporated anonymously when shared with the coach and Coaches Committee. Parent name is optional. If you wish to be entered into the draw for a chance to win $100, you will need to include your name. The winner will be announced on April 7, 2020. The winner will be notified and issued $100.

Completing an evaluation form is important as feedback is one of the tools used when evaluating future coaching applications so please complete the form honestly and accurately. The online evaluation form will be open until March 15, 2020 and can be found here.
