Final Registration Dates

As there is no general meeting in July,  the registrar (Dan Rudorfer) will be having a registration on  Tuesday July 15th from 7:00-9:00 at the pyramid centre

The only other time available will be just before (6:45-7:00) the August 5th SMMHA monthly meeting

    ABSOLUTELY NO REGISTRATIONS ACCEPTED DURING TRYOUTS.


Unfortunately we cannot guarantee that there will be any additional time for you to register your children for hockey before the deadline.  Forms are not required.

He will have your information for you to check and make any necessary changes. Simply bring your cheque book.

Rates:
Mite  -        2009, 2010, 2011                             $365.00
Tyke  -        2008 and first time 2007 players         $ 520.00
Novice  -     2006 & 2007                                    $ 635.00
Atom -        2004 & 2005                                    $ 635.00
PeeWee -   2002 & 2003                                     $ 660.00   
Bantam -   2000 & 2001                                      $ 660.00   
Midget-    1997,1998, 1999                                 $ 660.00
Rec League - 1995, 1996, 1997.                           $ 400.00
Family of 3                                                        $ 1465.00
Family of 4                                                        $ 1800.00

·         New this year: cost includes your book of tickets (1 book for each player)  


Registration cheques are to be dated Sept 01/14 (09/01/14)  or can be split into payments dated Sept 1st and Oct 1st 2014. A separate post dated to Mar 31/15 bond cheque is also required.  $50.00 per player or $100 per family 

New players require a photo copy of a birth certificate (OMHA only accepts birth certificates)  Statements of live birth and health cards are NOT accepted and unfortunately registration will be denied.  A photo copier will be available at registration.
