Police Background Check
Submitted By Pete Frehner on Friday, July 19, 2013
To comply with OMHA/OWHA requirements, everyone on a team staff that will be on the bench or going into the dressing rooms will require a police background check.

Here is the details on the process:

  • Obtain the Vulnerable Sector Screen form and authorization Letter from OMHA REP
  • Take letter and completed form with 2 pieces of ID to Sebringville during regular business hours.  Be sure to sign the form when you get there
  • Once OPP have completed their process (2-3 weeks) you will need to pickup your report from Sebringville
  • Place the completed report in a sealed envelope and deposit in the specially marked mail slot under the stairs at the arena.

THERE IS NO CHARGE FOR THIS PROCESS
